You have a patient that you want to switch from chronic daily IV meperidine of 50 mg to some oral form of analgesia as you want to discharge the patient. Here we go, and you can choose hydrocodone as an example and the tool calculates for you the equivalent dose with 33% reduction in calculation because of incomplete cross tolerance.
Just another tool from Epocrates that allow inter conversion of equanalgesic doses, something which we usually need in children on chronic pain medications.
This tool comes free with the other black bag "Med Tools" bundle of Epocrates but not as a stand alone, it should be attached to their other packages such as the Epocrates Rx (free) or Epocrates Essentials.
The tool is easy to use with the ability to even change the conversion factor if you think it is incorrect.
Also you can check up to three different opiates to convert to a final opiate with it's equivalent Fantanyl patch strength!
Even the issue of incomplete cross tolerance (Patients may not be as tolerant to the new opiate compared to the older chronic opiate) is also adjusted for.
Certainly a great product specially for pediatric oncologists and intensivists.
